Normal Dose

Susceptible strains of staphylococcal or streptococcal endocarditis 12-18 million units/day divide dose Q4 hours for 4 weeks $11.46/day
Susceptible strains of enterococcal endocarditis 18-30 million units/day divide dose Q4 hours for 4-6 weeks with an aminoglycoside $11.46/day
Gas gangrene 1-2 million units Q2-3 hours $22.92/day
Pregnant women-Group B Streptococci 5 million units followed by 2.5 million units Q4 hours until delivery $11.46/day
Late syphilis 2-4 million units Q4 hours for 10 days $11.46/day
Anthrax 5 million units/day given in divided doses until the infection is cured  



Renal Impairment

 

Creatinine Clearance Dose and Frequency
Crcl >50 ml/min no dosage reduction is necessary
Crcl 10-50 ml/min reduce dose to 75% of the normal dose at the usual dosing interval
Crcl <10 ml/min reduce dose to 20% to 50% of the normal dose at the usual dosing interval



Hemodialysis
