Dose and Frequency

For patients with a creatinine clearance of 5 ml/min or less should not receive imipenem and cilastatin sodium IV unless hemodialysis is instituted with 48 hours

A supplemental dose of the drug should be given after each dialysis period and at 12- hour intervals thereafter
