Your patient has been identified as being able to swallow PO dosage forms. The CIPRO IV that is ordered for your patient can be safely switched to PO with identical area under the curve (AUC) equivalency.

400 mg IV is bioequivalent to 500 mg PO Q12h

200 mg IV is bioequivalent to 250 mg PO Q12h

Patients with severe or life-threathening illness are suggested to be switched to the following:

**750 mg PO Q12h

(when severity of illness can be classified as mild/moderate, a switch to the *500 mg q12h is possible)
